Pain along the outer edge of your foot can come from several different sources — some gradual, some from a specific injury. Here’s how a podiatrist tells them apart and what to do about each.
Pain on the outside of the foot is one of the more commonly misdiagnosed complaints I see in my San Jose, Los Gatos, and Mountain View offices — not because it’s difficult to evaluate, but because several different conditions share that same general location. A peroneal tendon problem, a Jones fracture, and a cuboid issue can all produce pain along the outer foot, yet each needs a different treatment approach. This guide walks through the most likely causes, how each one typically presents, and what to look for when deciding whether to see a podiatrist.
The Anatomy of Lateral Foot Pain
The outside (lateral) border of the foot is home to several important structures: the fifth metatarsal bone, the peroneal tendons, the cuboid bone, and the lateral ankle ligaments. Pain in this region can originate from any one of these structures, which is why location alone — “the outside of my foot” — is a starting point for diagnosis, not a complete answer.
Two questions narrow it down considerably: Did it start after a specific injury or gradually on its own? And exactly where along the outer foot does it hurt most? The answers point toward very different diagnoses.
The Most Common Causes
Lateral foot pain has several possible origins. These are the conditions I see most often in my San Jose, Los Gatos, and Mountain View offices:
Peroneal Tendinopathy
Irritation or degeneration of the peroneal tendons running along the outer ankle and foot. The most common overuse cause of lateral foot pain.
Jones Fracture
A fracture at the base of the fifth metatarsal, in a zone with poor blood supply. Notoriously slow to heal and frequently missed as an ankle sprain.
5th Metatarsal Avulsion Fracture
A pull-off fracture at the fifth metatarsal base when the peroneal tendon yanks a bone fragment during an ankle roll. Common and generally heals well.
Cuboid Syndrome
Subtle subluxation or irritation of the cuboid bone, often following an ankle sprain or in high-arched feet. Produces midfoot lateral pain that’s easy to overlook.
Peroneal Tendon Tear
A partial or complete tear of the peroneal tendons, often from chronic tendinopathy or a forceful ankle injury. May require surgical repair in severe cases.
Bunionette (Tailor’s Bunion)
A prominence at the base of the fifth toe causing pain with shoe wear rather than activity-related pain in the midfoot or ankle region.
Sural Nerve Entrapment
Compression of the sural nerve along the outer lower leg and foot. Produces burning or shooting pain rather than the aching soreness of tendon problems.
Stress Fracture (5th Metatarsal)
A repetitive-stress crack through the fifth metatarsal shaft, most common in runners. Produces pain that worsens steadily with activity over days to weeks.
Pain at the outer ankle bone is different from pain at the bony bump on the outer foot, which is different from pain in the outer midfoot. Telling your podiatrist exactly where it hurts most — and pointing to the spot — significantly narrows the diagnosis before the exam even begins.
Peroneal Tendinopathy — The Most Common Overuse Cause
The two peroneal tendons (peroneus longus and peroneus brevis) run behind the outer ankle bone and continue along the outer edge of the foot. Their job is to stabilize the ankle during walking and to evert the foot. Overuse, a sudden increase in activity, or repeated ankle sprains can cause these tendons to become irritated, thickened, or partially torn over time.
What Peroneal Tendinopathy Feels Like
Patients typically describe a dull, aching pain along the outer ankle and the outer edge of the foot — sometimes extending toward the base of the fifth metatarsal. It is usually worse with activity, particularly walking on uneven ground or going up and down stairs, and may ease with rest only to return quickly when activity resumes.
High-arched feet place additional stress on the peroneals and are a significant risk factor. Runners, dancers, and people who have had repeated ankle sprains are also at elevated risk.
Jones Fracture & 5th Metatarsal Fractures
The fifth metatarsal is the most commonly fractured bone in the foot, and two different fracture patterns occur at its base:
Avulsion fracture — the peroneal tendon pulls off a small bone fragment from the very tip of the base during an ankle roll. Usually treated with a boot or cast; heals well with non-surgical care.
Jones fracture — a fracture slightly further down the shaft, in a zone with poor blood supply. Heals slowly, has a higher risk of non-union, and often requires surgical fixation in active patients or when healing fails.
Because it occurs during the same ankle-rolling mechanism as a lateral ankle sprain, many patients miss it without an X-ray. If you have significant outer foot pain after rolling your ankle — especially over the bony bump on the outer foot — an X-ray is essential before assuming it’s just a sprain.
Cuboid Syndrome
The cuboid is a small bone on the outer-midfoot that can become subtly shifted or irritated, typically after an ankle sprain or in patients with high arches. It’s one of the more underdiagnosed causes of persistent lateral midfoot pain because it doesn’t show up on standard X-rays.
Patients with cuboid syndrome typically describe a nagging pain in the outer-middle portion of the foot, sometimes with a sense of weakness or instability. It often develops after an ankle sprain that “just doesn’t get better the way it should.” Clinical examination, including a specific provocative test, is the main diagnostic tool.
Other Causes to Know
Beyond the most common diagnoses, these conditions can also produce pain on the outer foot:
- Peroneal tendon tear — a partial or full tear may produce persistent lateral ankle pain and weakness with ankle eversion. MRI is typically required for diagnosis.
- Bunionette (tailor’s bunion) — a bony bump at the base of the fifth toe that causes pain and rubbing with shoe wear rather than activity-driven midfoot pain.
- Sural nerve entrapment — the sural nerve runs close to the outer ankle, and irritation or compression produces burning or electric pain rather than the aching quality of tendon or bone problems.
- Fifth metatarsal stress fracture — in runners or high-activity patients, repetitive loading can cause a stress fracture through the fifth metatarsal shaft that worsens progressively without a single traumatic event.

How It Is Diagnosed
Diagnosis of lateral foot pain typically involves a combination of clinical history, physical examination, and targeted imaging:
Weight-bearing X-rays — the essential first step for any significant lateral foot pain, particularly after an injury, to identify or rule out fracture.
Clinical palpation — pressing specifically on the outer ankle, the peroneal tendons, the fifth metatarsal base, and the cuboid to identify the exact pain source.
Resisted eversion testing — pushing the foot outward against resistance isolates the peroneal tendons and helps identify tendinopathy or tear.
MRI — ordered when a peroneal tendon tear, occult fracture, or soft tissue injury is suspected and X-rays are normal.
CT scan — sometimes used to better characterize fracture patterns, particularly for Jones fractures where healing potential is a concern.
Cuboid provocation test — a specific clinical maneuver that can reproduce cuboid syndrome pain and support the diagnosis when imaging is unrevealing.
Treatment Options
Treatment for lateral foot pain follows two priorities simultaneously: accurate diagnosis first, then targeted management of the specific cause.
Rest and activity modification during acute flare-ups · supportive footwear and custom orthotics to offload the outer foot · physical therapy for peroneal strengthening and proprioception · walking boot for stress fractures or acute peroneal tendon injuries · cuboid manipulation by a trained practitioner · anti-inflammatory medications for short-term symptom control.
Jones fractures with delayed healing or in active patients who cannot afford a prolonged non-union · significant peroneal tendon tears that don’t respond to conservative care · bunionette correction when shoe modifications and padding no longer control pain. Most lateral foot conditions, however, respond well to non-surgical management when diagnosed correctly.

Most outer foot pain benefits from professional evaluation — particularly because fractures and tendon tears can mimic each other and require very different management. Don’t wait on these signs:

An X-ray is the most reliable way to distinguish them, and a podiatrist can examine you and order imaging the same day.
Cuboid syndrome is subtle subluxation or irritation of the cuboid bone in the outer midfoot, often following an ankle sprain or in high-arched feet. It produces a nagging midfoot pain that doesn’t show on standard X-rays, making clinical examination the key to diagnosis. It responds well to specific manual manipulation when caught and treated appropriately.
Jones fractures, while not life-threatening, can progress to non-union (failure to heal) if missed or undertreated — a complication that may ultimately require surgery. Peroneal tendon tears, if left unaddressed, can progress from partial to complete. Early evaluation prevents these more serious outcomes.
Not always. Many causes of lateral foot pain — including avulsion fractures and cuboid syndrome — are diagnosed clinically and with standard X-rays. MRI is most helpful when a peroneal tendon tear is suspected, when X-rays are normal but pain persists, or when more detailed soft tissue assessment is needed.
